After additional investigation, it was determined that security points had been the perpetrator behind the vapor cloud coming into contact with liquid hydrocarbons. Of course, the leak didn_t simply happen. Rather, it was the result of a sequence of events, each of which could have been prevented. Telos, the consulting agency hired to research the explosion, determined that minor things like falling blocks of concrete, thinned pipes, workers being subjected to fumes, and damaged alarms all contributed to the disaster.
